LISTA DE VERIFICACION PARA UNA EXPEDICION RADIACTIVA
(DANGEROUS GOODS CHECKLIST FOR A RADIOACTIVE SHIPMENT)

Se entiende que la lista de verificacion recomendada que aparece a continuacion, es para verificar los embarques en origen
( The recommended checklist appearing on the following pages is intended to verify shipments al origin)
nunca acepte o rechacee un embarque MORDAZA de verificar cada uno de los puntos.
(never accept or refuse a shipment before all items have been checked)

15. Una descripcion de la forma fisica o quimica, forma especial o material de baja dispersion.
A description of the physical and chemimcal form, Special form, or low dispensable material.
5. Se muestra el numero de paginas.

6. El MORDAZA de avion no aplicable fue borrado.
(The non-applicable Aircraft Type deleted)
7. Si el nombre completo del aeropuerto o MORDAZA de Salida o de destino no estan mostrafdos, anotelos.
(If the full name of Airport or City of departure or Destination is not shown, enter it).
16. El numero y MORDAZA de bultos y la actividad en bequerelios o sus multiplos en cada bulto o, para la Sustancia Fisionable, en lugar de la actividad, puede mostrarse el peso total en gramos o en kilogramos
The number and type of packages and the activity in bequerel or multiples thereof in each package. For Fissile Material the total weight in grams or kilograms of fissile material may be shown in place of activity.
8. La palabra "No Radiocativo" fue borrada.

17. Para radionucleidos individuales diferentes, la cantidad de cada radionucleido y las palabras: "All packed in one".

9. El numero ONU o ID precedidos por el prefijo ONU.
(UN o ID number, preceed by prefix).
10. El nombre apropiado de expedicion y el nombre tecnico, entre parentesis, para las entradas con asterisco.
(Proper Shipper name and the thecnical neme in brackets for asterisked entries)
18. La actividad dentro de los limites para bultos del MORDAZA A, MORDAZA B o MORDAZA C. (Ver el certificado de la autoridad compe tente)
